Depending on your health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.

I’d recommend asking these questions to your insurance provider to help determine your benefits:

  • Does my health insurance plan include mental health benefits?

  • Do I have a deductible? If so, what is it and have I met it yet?

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

 

What different insurances are you paneled with?

Yes. I am on paneled with Blue Cross & Blue Shield of Oklahoma, United Health Care, and Sooner-care/Medicaid. I also work with Medica Insurance and various Employee Assistance Programs. I can be found on the website as an approved provider for the affordable care act and individual insurance companies previously mentioned. I am diligently working to contract and negotiate services covered with other insurance providers. If you have an insurance carrier that I am not paneled with at this time, I do provide receipts that allow you to submit for reimbursement, IF YOUR PLAN ALLOWS IT. If you are unsure what your plan allows, I strongly recommend you call your insurance provider to inquire on the reimbursement options.

To assist, you may provide them the following CPT Codes listed on our receipts.

  • 90791 - Initial Diagnostic Evaluation (assessment)

  • 90837 - Individual Psychotherapy (individual appointments)

If your insurance requests a Diagnosis Code, I frequently use the below codes; your actual code may differ based on assessment during intake depending on your specific circumstances.

    Payments

    All co-pays are due at time of service. I accept all major credit cards as form of payment. I also accept payments thru Paypal, & Square as well as cash, check & money orders.
